This is an ideal tool for professionals—insurance agents and brokers, health care professionals, business owners, social workers, legislators, teachers, librarians and consumers—who have a basic knowledge of health insurance and need a quick-reference format.



Application and Enrollment Database

The Application and Enrollment Database provides state by state applications to sign up for private and public health coverage. Some programs have applications you can print and some have links to Web sites where you can apply or get a quote. For all others, you can use the Health Care Options Matrix™ Guide or Web tool to locate contact information. You may also call the U.S. Uninsured Help Line at 800-234-1317 for assistance.

Plans for Individuals Recently Covered by an Employer Health Plan:

COBRA or state continuation coverage: Your employer will provide you with an Election Form that you must sign in order to continue receiving benefits. You may also need to begin paying the monthly premium at that time as well.
HIPP: You must apply for Medicaid to see if the state will pay your COBRA premium. See Public Programs above.
HIPAA: Obtain a Certificate of Coverage from your group health plan which you will then present to the new health plan.

The Help Line is a phone service designed to help you find free and low-cost health insurance. Available 24 hours a day, seven days a week, in multiple languages, the service is staffed by friendly Information Specialists who can provide callers with three main services. They can:

  1. describe the public or private options for which the caller may be eligible.
  2. connect a caller to representatives from various plans.
  3. help find special resources pertaining to a caller’s unique health insurance eligibility situation.
